The following pages contain a survey that will help us to create your personalized materials for your STEPS appointment.  The survey includes questions about you, your use of alcohol, your attitudes and expectations about drinking, things you do to keep yourself safe when drinking, and experiences you might have had associated with drinking.  Please answer the questions as honestly as possible so that we can make sure that your STEPS materials are relevant to you.
