Welcome to STEPS 2.0!
The following pages contain a survey that will help us to create your personalized materials for your STEPS 2.0 appointment. The survey includes questions about you, your use of alcohol and other drugs, your reasons for using cannabis and/or alcohol, things you do to keep yourself safe when using these substances, and experiences you might have had associated with cannabis or alcohol use. Please answer the questions as honestly as possible so that we can make sure that your STEPS 2.0 materials are relevant to you.
Your responses to the survey (including those about illicit substance use) are confidential and will not be shared with anyone else.
